The data provided in the “Student Enrollment Details” will be used for class roster inclusion, important class communications, Certificate of Completion issuance, and, if applicable, Continuing Education (CE) hour reporting.

Each student must have a unique email address from other students enrolled in the class to access their portal; please do not share email addresses. If you experience any issues, please contact our office for assistance.

Our payment processor requires the cardholder’s name, credit card number, billing address, and security code to match their records for payment authorization. 

If a third-party is making payment and “Billing Details” differ from the Student Enrollment Details, please check the box below.
